Lethal Subarachnoid Hemorrhage after Successful Endovascular Embolization of a Traumatic Carotid Cavernous Fistula: a complication report

چکیده
A 35-year-old male experienced the rapid development of left pulsatile exophthalmos and chemosis in the early course after craniofacial trauma. A high-flow direct carotid-cavernous fistula was found at diagnostic carotid angiography. After successful endovascular embolization of the fistula by balloon and coils, the patient developed a diffuse subarachnoid hemorrhage leading to death. We postulate that re-damage of a previously injured intradural cerebral artery by increased cerebral blood flow after obliteration of the fistula resulted in the lethal diffuse subarachnoid

Endovascular targeted coil embolization using dual vessel approach for traumatic direct carotid- cavernous fistula following severe head trauma: technical notes
Direct carotid cavernous fistulas (dCCFs) represent different entitles from spontaneous cavernous sinus (CS) dural arteriovenous fistulas (CSAVFs). Traumatic dCCFs are direct arteriovenous communications between the internal carotid artery (ICA) and cavernous sinus, resulting from a traumatic tear of the horizontal or posterior ascending intracavernous segment of the ICA by skull fracture fragm...
متن کاملCarotid cavernous fistula after percutaneous balloon compression for trigeminal neuralgia: Endovascular treatment with coils
BACKGROUND Percutaneous ganglyolysis treatment of trigeminal neuralgia is rarely associated with vascular complications, such as hematoma, subarachnoid hemorrhage, and stroke. Internal carotid artery injury may also occur after misguided needle placement, particularly far posteriorly or medially, resulting in carotid cavernous fistula. Anatomical variations of the foramen ovale can predispose t...
متن کاملSimultaneous subarachnoid hemorrhage and carotid cavernous fistula after rupture of a paraclinoid aneurysm during balloon-assisted coil embolization.
We describe an iatrogenic perforation of a paraclinoid aneurysm during balloon-assisted coil embolization that resulted in simultaneous subarachnoid contrast extravasation and a carotid cavernous fistula. The causative factors specifically related to the balloon-assisted method that led to aneurysm rupture are discussed as well as strategies for dealing with this complication.

This case report demonstrates delayed rupture of a cavernous carotid fistula and aneurysm into the temporal lobe 12 years after treating a direct cavernous carotid artery fistula using detachable silicon balloons. The ultimate treatment was performed using arterial endovascular sacrifice. Successful treatment of cavernous carotid fistulas may ultimately lead to formation of cavernous aneurysms....
متن کاملCarotid cavernous fistula due to a ruptured intracavernous aneurysm of the internal carotid artery: treatment with selective endovascular occlusion of the aneurysm.
Intracavernous carotid artery aneurysms causing a carotid-cavernous fistula (CCF) are rare. These aneurysms usually cause neurological symptoms due to gradual expansion without rupture. If they do rupture they most often lead to a CCF instead of bleeding into the subarachnoid space. A patient is described with a ruptured intracavernous aneurysm causing a CCF resulting in acute onset of unilater...
